Cancel Registration

Cancel Your Voter Registration

Use this form to cancel your voter registration. Upon receipt of the form, the county board of elections will remove you from the county’s list of registered voters.

Report a Deceased Voter

Use this form to notify a county board of elections of the death of a North Carolina registered voter. Upon receipt of the form, the county board of elections will remove the voter from the county’s list of registered voters. Only a near relative or personal representative of the deceased voter’s estate may complete the form.
